Medical Leave of Absence Form

The Medical Leave of Absence form is intended for students requesting a ‌ (MLOA) from ÁùºÏ±¦¿â and, at the student’s discretion, may be provided to any one of the following individuals:

1. The Associate Dean of the College in which the student has most recently enrolled (“Associate Dean”)
2. The ÁùºÏ±¦¿â Counseling Center (“Counseling Center”)
3. The ÁùºÏ±¦¿â Student Health Center (“Health Center”)
